Integrating evidence-based interventions into the workflow to transform clinical practice

Recognizing the hurdles of implementing quality assurance performance measures in the ambulatory setting, Zynx Health developed Zynx AmbulatoryCare to improve adherence to best practices and performance measures specifically for primary care physicians at the outpatient level. With over 100 diseases/conditions modules, Zynx AmbulatoryCare addresses many high-volume, high-profile conditions (eg, Diabetes Mellitus, Dyslipidemia, and Hypertension).
Each module is divided into six sections to facilitate swift access to in-depth reviews of the latest clinical research, current performance measures, and point-of-care clinical decision support:

  • Evidence Summaries contain concise synopses of the peer-reviewed literature with links to abstracts and full-text articles (when available) for more comprehensive literature reviews.
  • Forecasting Functions help you model the impact of evidence-based interventions on clinical outcomes.
  • Evidence-Based Order Sets have been created exclusively for the ambulatory setting and include links to clinical summaries, regulatory standards, and national pay-for-performance measures. Using the online editing tool, order sets can be easily customized for local best practices, clinical standards, and formularies.
  • Reminders and Alerts drive point-of-care adherence to regulatory measures and pay-for-performance guidelines.
  • Performance Measure Checklists provide a current overview of national performance measures along with easy-to-use guides to specific interventions for target patient populations.
  • Continuing Medical Education (CME) credits are available within selected modules.
